Examples of our key programs that meet the needs of children and youth in foster care include:
A coach-driven, wraparound case management model that establishes a comprehensive network of health and social supports around the child—including health, community resources, child welfare, housing, employment, financial, and other key resources in the community to help them stay healthy and successful.
A program to provide youth aging out of foster care with the knowledge and skills to navigate and access their health care services, improve life skills and confidence, increase connectedness with at least one identified adult support, and eventually live self-sufficiently—all leading to better health and quality of life outcomes.
